My recommendation after 18 years of work with kids is whole milk should
never go in the bottle after it is mixed half and half. If they learn that
milk won't come from the bottle, but only the cup they tend to more quickly
dump the bottle. You just make sippy cups appear with milk, and either make
the bottles go away, or fill them with nothing but water. Some kids need a
few different sippy cups to find the one they prefer, but quitting the
bottle cold turkey works the best for sure. They try to hold out and don't
drink quite as much for a few days, but they do just fine. Good luck it
could be a fussy few days, but some kids just make the switch like magic.

We use the liquid formula. It is more expensive, and Wic has been providing
it for the past few months, but even when we weren't getting it from Wic we
thought it was well worth it because it was much more convenient. We kept
the powdered formula for if we ran out of the liquid or if we had to go out
for a long period of time. But we're starting Kahlan on whole milk as of
yesterday, so now she's getting two ounces of milk and six ounces of formula
if she has an eight ounce bottle. She'll be exclusively on whole milk by
next month, I think.
Then I'll get her off the bottle and on to the sippy cup exclusively,
hopefully shortly after that.
