Thanks to all for your excellent ideas.  I too use to practice with water but some medications had much more viscosity than others so I would drip the medication on the end of my finger to count and let it fall into a container that I would have my daughter drink.  
In the drop measuring phase I would pour the medication back in the bottle and so on.  That did require having a small funnel.
This request was the result of some engineers in Israel that we work with.  They wanted to create an electronic device that would solve the problem with audible feedback.  Although I think this is a workable solution, it would likely be expensive and require lots of testing so I thought I should forward to them the simple solutions blind parents are already using.

Depending on the thickness of the liquid medication, on average one drop equals 0.05ml. Four drops would be 0.2ml. A pharmacist may be able to mark a syringe for you, by placing a small cut or piece of tape on the plunger part of a syringe. I test the medication dropper to see how much pressure I need to apply to squeeze out one drop by drawing up water in the syringe & squeezing the bulb end of the syringe & letting the drops of water drop into the palm of my free hand. Once my children were able to count, I would practice using a medication dropper filled with water & have the child count the drops out loud, pausing between each squeeze.
