Interesting. Looks pretty accessible and straightforward here, although God only knows how the individual airlines can and will muck up the works. This form provides for owner-trained service dogs and last-minute travel. A step in the right direction? I’d like to see the forms eliminated altogether, but I’m not sure that’s going to happen, given the problems we’d had lately with fake service animals.

Hello Chapter Presidents,

As I mentioned at this morning's meeting, here is the new US Department of Transportation (DOT)
Service Animal Form; its attached and linked below.
Two key ways to know you have the right form 1) the new one expires on 9/30/2027 and 2) visually the DOT logo has moved to be left justified whereas before the logo was centered, so visually it's obvious if you have the old form or the current one.

If the airline has a service animal form on their website then this PDF
version is not needed, so it depends on the airline if you use this form or
their website--check with the airline.
