[CT-NFB] Question about form for free currency reader

Federation friends and family,
I received the below link from a family member and although it says the application needs to be mailed and could not be submitted electronically, the online form is described as a fillable PDF. When I press the button for English, it brings me to the form however, I cannot get the virtual keyboard to come up on my iPad anywhere within the form. Any suggestions on how I might get my iPad Pro to fill this form out non-visually?
