How about a large print check register? They are available at several 
sources. You might even fill in food notes or other comments on the long 
section of the line.

There are wide ruled black line tablets, too.  The lines are even farther 
apart on those giving you room for larger print. . You would have to make 
columns with a ruler and dark or bright colored marker, but since the pages 
are 8 and a half by 11 the ruler will extend the full vertical length of the 
paper.

My insurance company requires a log essentially every 30 days, so I use an
excel spread sheet. I send mine to my provider every month.

I found a way--use Excel and label your column headings and in cell A
put the dat followed in the next cell with the time and in the third
cell put your reading.

Print before going to see your doctor and this would help the doctor
see how you are doing between visits. An A1C test only covers about
90-days and if you don't see the doctor every three months this would
be helpful for managing your diabetes.
