I think it's time for your client to accept his current state and get on 
with life.
Why doesn't he try attaching something to the device and hold it in his 
teeth?  I know that's contradicting my above statement, but I thought I'd 
float it out there.
And Tony, a lanyard isn't good.  They aren't stiff.
I think the stand for harmonicas may be to much added equipment for your 
client to carry with him on fishing trips.
