I think Cheez is right. To go to the top of a document, you can
double tap the status bar, where the clock, battery level, signal
strength, etc are.
There is no easy way to get to the bottom of a message. I will often
tripple click home to turn off voiceover and then use my finger to
scroll a lot. Then turn voiceover back on to see if I've gotten to
the bottom.
