I've gone through CPR and First Aid a few times now (since it has to be renewed every so often). I've never really worried about telling the instructor a head of time about my blindness. The instructors I've had in the past were fine. There was a bit of an issue with one exam, but it was more of a classroom control issue than anything having to do with blindness (one of these days Ronza or I will have to tell you that little gem off list).
Generally, instructors will have to do some hands on work to help others with their hand positioning and technique, so working with you shouldn't be too out of the ordinary for them. I would consider how you will take the written exam. I have done it two ways: scribing my own answers on a laptop for the instructor to grade if they read the exam out loud, or taking the exam with the instructor scribing for me. It just depends on the environment.
Are you taking a Red Cross or AHA course? I believe BookShare has the materials for at least the Red Cross course in an accessible format if you would like to review the workbook prior to class, or bring it with you.
