Dear All,    
I thought someone on the Human Services list could help me. I am trying to resolve an issue between a guide dog training program and one of their consumers. The program is asserting confidentiality and refusing to speak with me concerning this issue, so I need a release of information form for the consumer to send to them. If anyone has such a form that I could use/edit for this purpose, please send it to me off-list to
