Have you tried a 3-way call with the consumer and the school?  Shouldn't be
a concern about confidentiality at that point, and they can get it in
writing that the consumer is giving you verbal permission to discuss the
issue with their school.  Just a thought,

I thought someone on the Human Services list could help me. I am trying to
resolve an issue between a guide dog training program and one of their
consumers. The program is asserting confidentiality and refusing to speak
with me concerning this issue, so I need a release of information form for
the consumer to send to them. If anyone has such a form that I could
use/edit for this purpose, please send it to me off-list to
