Your point about treating folks outside your licensed state is an important 
consideration.  Another important consideration is your malpractice 
insurance, make sure that you are covered for cyber treatment as the 
coverage may be different.

@ Geraldo: When you request a payment through Pay pal it gives you the
option to choose the currency you are requesting. However I have to warn you
that  you are putting your license at risk  to counsel anywhere other than
your own state.
