[humanser] requesting a digitally formated evaluation template

Hello All, 

Recently, I volunteered to participate in a program. The purpose of the program is to have Peer Support Specialists join a small team of professionals tasked with completing various evaluations when visiting independent living homes. It may or may not be relavent that I will receive a stipend for the visitation and the completion of the evaluation. 

I have not outed myself as a blind person. Obviously, I need to self-identify when I request the evaluation template in a digital format. I have a few questions. 

How should I explain what I need? For example, I'll need to mark check boxess, use a rating scale and fill in comment sections and facility information. Also, I'd like to use my personal iPad to complete this task. Does HIPPA preclude me from using my iPad and submitting my evluation via E-mail? Does the fact that the agency will provide a stipend put me  in a catagory other than volunteer? I ask this particular question because organizations are NOT required to accomidate volunteers.
