As an addition to the major conditions identified in the DSM 5 the attached 
are additional codes that further identify the condition.  These are, for 
the most part, what used to be Axis IV under the DSM IVTR.

I found it on page 217 of the DSM-5. It is 300.22. I could not copy
it, but I am working on it.
