Hey,
What I do is I have a template of what is needed by the agency I work 
for. We started by using a SOAP note format and recentely switched to a 
DAP note format. I have the template set up and I copy and paste the 
diagnosis, issues and other pertinent information in and I am set when 
the client shows up. I type in what happens in the session and am done. 
It works great for me.
