CASE MANAGER 

 

For Adults who are Blind or Visually Impaired

 

Description: This full-time position provides a wide range of case management and related services, including scheduling doctor and other health-related appointments, arranging and (periodically) providing transportation to and from appointments; supporting the selection of insurance plans; applying for public entitlements such as PACE, Chicago’s Taxi Access Program, and other transportation services; processing applications to the agency’s various program, such as the Rent Assistance Program; and providing other case management and related services.  

 

This position is ordinarily based at Friedman Place but for the foreseeable future will require orientation and training on-site and then transition to all or mostly remote. Providing transportation is not likely until after the pandemic is over and then, only periodically and for short distances. 

 

Compensation: $16 - $18 per hour, depending on skills and experience. Friedman Place provides very generous benefits including 20 paid days off plus 6 paid holidays to new full-time employees; heavily subsidized health insurance (employees pay $23.08 per pay period for PPO and $6.92 for HMO); retirement contributions (after 2 years); educational and licensure reimbursement;  discounts on products and services; flexible spending account, discounted meals, etc.

 

Primary Qualifications 

·         Bachelor’s Degree in Social Work or related field - will consider masters-level as well 

·         At least two years case management or related experience

·         Experience working with people who are visually impaired and/or disabled a plus

·         Proficiency in office computer programs, including Microsoft Office Suite, etc. 

·         Excellent written and oral communication skills and the ability to work effectively with diverse individuals and groups. Very high level of organizational and time-management skills and other skills and abilities necessary to provide the responsibilities and requirements outlined above.

·         Valid driver’s license
