The National Center on Physical Activity and Disability is looking for individuals with disabilities to star in video clips demonstrating a wide variety of exercises and fitness activities. These clips are for use in online (i.e. YouTube) programming, including NCPAD’s annual 14 Weeks to a Healthier You program (http://www.ncpad.org/programming/fact_sheet.php?sheet=677) that begins in March.

Filming will take place during business hours in the DHD Building at the University of Illinois at Chicago at 1640 W. Roosevelt Road during the month of February, and possibly beyond if interested. Please contact Blythe Hiss at 312-996-5965 or sbonne2 at uic.edu if interested. No previous experience required and a small honorarium is available.

1. Do you have a disability? If so, please list/describe.
2. Do you use an assistive device?
3. Do you have any activity limitations?
4. What is your age?
5. What is your ethnicity?
6. What is your availability during weekdays?
7. Do you have transportation (to 1640 W Roosevelt)?

8. Is there a phone number I can use to contact you?
