Hello,

We have been invited by the Chicago Mayor's Office for People with
Disabilities to participate in a survey focused on obtaining the public's
perspective on the City of Chicago's Five-Year Plan on Homelessness in our
city. All who live, work, or study in Chicago are invited to respond.
