Hello,

I am reaching out to ask if you can share a survey with individuals at your
organization. I am a doctoral candidate researching practices in disability
services. I am comparing how staff and students feel about common practices,
what disability models are present, and how supportive each practice and
service is to a student's education.

I have identified your organization as one that may be able to share the
survey with individuals who attended a college or university in the United
States between 2012 and now. If you are willing to share the survey link, I
am asking for participants who have worked with disability services staff
for at least a semester. Participation is voluntary and results will be
confidential. The survey should take about 15 minutes.

For participants, possible discomforts or risks include possible feelings of
discomfort or anxiety recalling information about their experience in higher
education. There may be risks the researchers have not thought of.

I will not collect any identifying information (like your name, IP address,
etc.). Participants will have an opportunity to provide an email address to
me if they would like to discuss their experiences further in an interview,
but that is voluntary and is in a separate form. The data will be kept in a
secure location on a password-protected computer. Only I will have access to
the data.

Participants have a choice about being in this study. They do not have to be
in this study if they do not want to be. They can exit the survey at any
time.

Participants may have questions about your rights as someone in this study.
