Hi Everyone, 
This is a state program and could be used for transportation and/or reader services. The application is about 12 pages long with all kinds of questions. 
Attendant Care Program

 

What is the Attendant Care Program?

The Attendant Care Program is designed to encourage individuals with severe chronic or permanent physical disabilities to direct their attendant care services. The Program provides financial reimbursement to supplement individuals' costs of attendant care services. Services may include in-home assistance with personal care, chore services, and transportation. The participant is reimbursed a percentage of attendant care costs based on annual income, household size, and available Program funding.

Who is eligible for services?



Applicants must meet ALL of the following requirements:

 

Ø      Severe chronic or permanent physical disabilities 

Ø      Maryland resident

Ø      Between the ages of 18 and 64

Ø      With a total gross income of less than $53,522 per year

 

----------AND AT LEAST ONE OF THE FOLLOWING:----------

 

Ø      Currently employed; or

Ø      Actively seeking employment; or

Ø      Currently enrolled in an institution of post secondary or higher education; or

Ø      A nursing facility resident, but could reside in the community if attendant care service is provided; or

Ø      Living in the community, but at risk of nursing facility placement if attendant care service is not provided
