Yasmin,

Thanks so much for your interest in participating in my research on the SSI/SSDI application process! This is a unique study that will look at how we understand and interpret the forms used by SSA and how these may be revised. Participation will include two one-hour focus groups. The first will take place between September and October, and the second between January and February.
Attached is a short form that I need you to fill out and email back to me with your contact and demographic information. It also asks what days/times are best for you to meet. I will schedule the group meetings accordingly, so be sure to list any times that would not work for you. I will contact everyone in the next 2-3 weeks about their participation. Please feel free to pass this form on to anyone you think may be interested!
