The purpose of fingering is to make piano easier to play.  Do what works most comfortably for you, remembering that the black keys are easier to play with the index, tall and ring fingers since they are set far back and the thumb and pinky finger are short. Your hand should be in a relaxed position with the wrist on the same plane as the keys so that you don't twist your hand.

How did you learn how to play melody and harmony with the right hand.  I am using a play by ear and emprovise course set that I got from the music section, and the instructor demonstrates how to play chords and melody with the right hand on it, but she doesn't give finger suggestions at the beginning of the piece.  Do I just improvise it in the way I can play it, or follow her instructions?

I'd recommend going to Hadley, and taking lessons there, so they can show you the fingering.
