[nabs-l] creating surveys

if you use this service, what format can you put the survey in? Multiple 
choice?
How do you get people to take it? do they need a password?
Also, how do you  retrieve results? Will it analyze results for you?

I recently used surveymonkey.com to evaluate my college course.
the service allows you to create a survey comprising of 10 questions (free) 
or unlimited questions (subscription).
Service is completely accessible and you can embed the survey on your blog. 
share on social networks, email, or web link.
There is another service called poledaddy, but I haven't used this 
personally.
