Hi Ashley,

It does everything for you. 
You begin by giving your survey a title. 
Then you input the text of your questions. 
You can choose from a wide range of answers, i.e. rating scale, multiple choice, drop down menus, essay or comment style text boxes, and others which I haven't explored. 
You can configure the way individuals take your survey, but in the case of mine, my class mates just clicked on the link, filled in the survey, and clicked done. 
The results are col'lected and analyzed by surveymonkey, you can view these results at anytime (and view the number of responses).

In your case, you could create your survey, embed it on your blog, and then tell your participants to visit your blog.
Individual links can be sent to individual participants via email.
