Hi Ahbee,

You could bring a sighted friend to the doctor’s office to help you with the paperwork or if you’re comfortable enough, perhaps Be My Eyes, AIRA, or any other reader could help you with that. The best option in my opinion is to have the paperwork emailed to you so you can have them in a digital format and it should be accessible for you to view and fill out.
