Hi Laurie,

That was why I suggested items from, say, the school's gift shop, or
something to that effect. Yes, the school has to purchase these  items. But
often they will sell them to grads at a discount anyway. I don't see how
this is less fiscally responsible than increasing the amount of veterinary
support available to people who fill out the forms. 

I still think it would be interesting if somehow a survey could be done to
find out why these forms are not filled out as often as the schools would
like. Perhaps if those issues could be addressed, incentives wouldn't be
necessary.

By nature, the guide dog programs are non-profits with dedicated budgets.
It seems to me that if a program wanted to offer insentives for something
like filling out health info (which, imho, a supporter of the program ought
to want to do willingly), the cost of these insentives would need to be kept
as low as possible.  If the insentive was something that the program could
donate from its own budget, instead of being something that had to be bought
from somewhere else, the cost would be the most reasonable.  
Getting into the business of purchasing gift certificates or other stuff to
hand out to get people to fill out forms just seems like a suspect business
decision to me.  /smile/
