Kathy,

I have recently started taking my guide dog to work with me.  Like Rox'e, I 
also owner train my guides.  I don't have "certification" of any sort.  He 
has been individually trained to mitigate my disability.  When I felt he was 
ready to accompany me to work, I went to my supervisor and presented him 
with my desire to have my guide dog there.  Fortunately I work with really 
great people.  It wasn't a problem for anyone.   My supervisor is also a 
lawyer, so he pretty much knows the rules about such things! *smile*

I'm not sure what sort of work you're looking for.   I'd say in most jobs 
the dog should be easily manageable by you in the manner you are accustomed 
or that seems obvious.  If you will be working in a sterile environment, in 
a food preparation area, in direct contact with other animals, around 
hazardous chemicals or in any situation where you are not comfortable 
working your dog, you may have to work out some details particular to your 
situation.

I work in an office setting in the courthouse, so he just mostly sleeps 
under my desk.  I do work him through the building when I need to go to 
other offices, but some people prefer to use their cane after arriving at 
work.  Really it's up to you to pick what works for you and express those 
needs to the employer.
