No I truly believe the problem was masked and they just didn't see it.  
They were pretty good vets and had state of the art equipment it just 
slipped by them.  If this can happen to health care professionals 
wouldn't it be reasonable to suppose an instructor who must be watching 
a great many things could easily miss something?
