I think it's fair to say that most people do not train their own service 
dogs.  It is, however, much more common among other disabilities than 
blindness.   If I had to take a guess I'd say that around 2% of blind 
people  have guide dogs trained through some other means than a 
traditional program.  It's a very small number, but I don't understand 
what the number of people in a particular group has to do with that 
group's civil rights?

And two quick things about ID cards...anyone with a basic knowledge of 
computers can make an ID card.  Second the card, even if from an 
established program, doesn't prove that the dog is well behaved.  I 
think we all know of dogs that should not have been issued as guides or 
those whose handler didn't keep up with the training.
