Sarah,

Have you thought of Guide Dogs of Texas?  They are a really small school 
serving only Texas.  I'm pretty sure they do exclusively in home 
training, but you'd need to confirm that with them.

A lot of the programs say that they will do in home placements if the 
situation warrants it.  I don't know what their criteria is, but you 
could always speak with them directly to find out.
