Highly recoomend the suggestion of a puppy raiser.
I had raisers look after my dog when I was having my daughter. It worked out very well.

A thouoght came to me - haha!  Are there any puppy raising groups near you?
Some friends here in San Diego had to be hospitalized and/or be in rehab for
several weeks and even months!  Here, people have had a puppy raiser take
care of the dog for the person in the hospital.  Another friend broke her
wrist and had to not use her dog for around six months.  The Seeing Eye took
the dog back for the time my friend recovered.  At the school, her dog got
taken home with a trainier, stayed in the kennel, and did get worked by an
instructor.  When my friend was able to use a dog again, TSE came back with
her dog and they had a few sessions at my friend's home to make sure their
work was fine. it worked out fine.

You might like to ask around the puppy group or your school.  I have never
left my dog with a hospital staff when I had surgery. he stayed with my
friend who broought me to the hospital and picked me up after surgery - i
only had day surgery.
