[nagdu] Seeking Guide dog Training agreements

When I got my seeing-eye dog there was paperwork saying you own your dog and you will pay $150 or $50 depending on which dog you received first or second. There was also a financial agreement saying I will pay when I come to class or I will pay in installments of X per month.
