Could be that the infection isn't healed.  It could be (and I 
hate to say this) but it could mean that you need to get a 
stronger antibiotic.  They have pill form ones that you may have 
to do.  Either that or there's an allergy issue, but I'd guess 
you need a stronger antibiotic, plus you'll have to clean his ear 
every day until the stronger antibiotic works.    Peace,    Debby
