Hi Pam, 
I use a hands-free lead, so it goes over my right shoulder, across my body and attaches to either my dog's flat collar or newtrix headcollar on my left.  The leash, at it's shortest is about 3.5 feet long, it can also be lengthen either part or all the way to a 7 foot leash.  I have it part way hooked because my current guidepup is short.  
In cafes, I usually just lower the strap from off my shoulder and let it lay around my waist.  This way I can feel if she gets up— usually never happens, or if she decides to try for some ground candy— happens from time to time, labs being what they are.
