I agree. Nothing like getting my attention like the title of this thread!
Holly very rarely does anything like that and when she does, its usually me
having forgotten to take her out.

I hope the vet does figure out something and can provide a resolution.
If not, it might just be from stress.
It really sounds like a bladder or kidney issue. He's an adult now, and he
should be able to hold his water until he gets outside. If he can't, there
certainly is a health issue the vet is missing.
If you can, visiting a veterinary chiropractor is a viable option.
However, we all have our limits on what we are willing to deal with.
Urinating indoors is understandably very burdensome.
