Sending a written response is a good idea.  Send it registered mail, so you
have a receipt and they can't say they never got it.

For all the reasons previously stated, I also would not sign this document.
I suggest sending a written response, outlining the reasons why this
document does not apply to your new guide dog.  

Particularly if the landlord is trying to add this form to your lease, I
suggest that your response be received well in advance of the termination of
the current lease.
