[NAGDU] Airline forms

The airlines have the information about their requirements on their pages about traveling with service animals. It's a good idea to check your airline's policy before actually making the reservation. No doubt it's a pain.

Since all airlines may not require the forms, is it easy to tell if the one I've chosen does?  

Frankly, I have a lot of trouble just finding the flight I want and getting that all set.  If the form isn't right up front, I'm liable to miss it.  By the time I've jumped through all the ordinary hoops, I'm in no mood to go hunting for some stupid paperwork.
