[NAGDU] airline survey link?

Hi all,

Could someone send me the link to the survey about dealing with the DOT paperwork, and questioning if we either choose not to get a dog or not to fly because of the forms? I saved the email with the link, but I didn't save it where I usually do and I can find it. A friend of mine would like to fill it out.
