Hello Marianne.
I may need additional information to provide a more concrete answer. Such
as, what's the nature of the job, and what prompted this employer to ask for
documentation. For example, is this a general form for all employees who ask
for a reasonable accommodation, or is it due to some other reason?
As a general rule, service dogs are not specifically defined in Title I of
the ADA. This means that when a person wants to bring their service dog,
A.K.A. guide dog to work, it falls under a reasonable accommodation. Since
reasonable accommodations for work are things that an employee can ask for,
the employer can ask for documentation to support the request for the
reasonable accommodation. As a general rule, when it's obvious that the
employee is blind and using the guide dog for mobility, an employer doesn't
ask for a justification. This is why I wonder what other details there may
be with this situation.

I am on another FaceBook group and the question is about completing an ADA
form to have a guide dog as a reasonable accommodation. Is this legal? Do we
need to justify why we use a guide dog rather than a cane?
