[NAGDU] guide dog as reasonable accomodation?

Every job I have had I have had to fill out a form, asking for specific reasonable accommodations and explaining why. I just state that I am blind and attach some kind of note from an ophthalmologist. But it has been my experience that a reasonable accommodation request form is pretty common practice at least in Hawaii. if this is not the case then maybe they or that company has never had a request for a guy dog before and if they are uneducated about dog purpose then just fill out the form. Or call human resources and ask to speak with their ADA person.
