[NAGDU] guide dog as reasonable accomodation?

Who is asking that this form be filled out? If it's for housing (as an example, an apartment complex?, It might be legal, particularly if you are moving into a pet free apartment complex. But folks with more legal experienced than me would know. You might also consult with your school.
