1.      What components should be included to produce a well-designed customer feedback survey and what should be included in them in order to obtain the best results? 
Answer #1.  Each question should contain a rating system for the respondent to use.  Questions should be designed as evaluation questions and not just either or questions.  Good surveys of this type are better if they are designed as accountability tools rather than satisfaction tools.
 
2.      Based on a community risk assessment, is the HFD providing proper services and education for all of its citizens including those identified bythe United States Fire Administration (USFA) as "at risk" target groups? 
Answer #2.  I have no idea, the risk assessment outcome has not been distributed widely enough for any opinion to be reached.
 
3.      How can the HFD better educate its constituents on the services itdoes provide and ultimately achieve a better rating when feedback surveys are ultimately completed for the department to review? 
Answer #3.  In-person surveys and presentations to people who are blind and groups of blind people are the strongest methods to reach people who are blind.
