I obtained your email from the website of the National Federation of the
Blind of Hawaii Anuenue Chapter@ http://hawaii.nfb.org/index.html. I am
presently a captain with the Honolulu Fire Department who is working on an
applied research project (ARP) for my department as a candidate in the
National Fire Academy's Executive Officer Program at Emmitsburg MD. As part
of my ARP which I have titled, "How Well Do Honolulu Firefighters Measure
Up?" I am asking for your assistance in helping me with three questions in
an attempt to find the answers to each of the questions below. They are:

1.      What components should be included to produce a well-designed
customer feedback survey and what should be included in them in order to
obtain the best results?

2.      Based on a community risk assessment, is the HFD providing proper
services and education for all of its citizens including those identified by
the United States Fire Administration (USFA) as "at risk" target groups?

3.      How can the HFD better educate its constituents on the services it
does provide and ultimately achieve a better rating when feedback surveys
are ultimately completed for the department to review?

At risk individuals as defined by the USFA (question #2) include deaf and
hard of hearing, blind and visually impaired, elderly (over 65), young
children (less than 5), and mobility impaired. As such, I am hoping you and
your chapter will help me answer each of the questions listed above as they
relate to individuals who are blind or visually impaired?
