I was able to take your survey.  I just arrowed down the page and pressed the spacebar to check or uncheck an item.  Number 30 asked us to rate if we strongly agree, agree, are neutral, and so on... This was not described with the screen reader but I responded by checking the 2nd one in the list hoping that I was checking agree.

I use Jaws 2018 .
