Thank you Colleen.  Yes, question 30 asks for rating of each sub-question
along the five-point scale arranged from left to right: Strongly agree;
Agree; Neutral; Disagree; Strongly disagree.

On our tests, JAWS 12 displayed the response options properly.  I am only a
little bit surprised to hear that JAWS 2018 doesn't do so.

What computer operating system, and what Internet browser are you using?
