I never thought of the idea!  I would measure the bible and maybe add an inch for movement.  Measure the hight.  Knit a flatt piece, knit a row, purl a row.  For it to lay flat I would think you would want it flat on the frunt.  Add 2 inches to each end for a flap to go over the edge of the book.
After making the piece, turn over 2 inches at each end for the covers to slide in.  You might also want to slip stitch on your turns.

Just an idea off the top of my head!
