[NFB-Utah] A research survey for the blind

[Nfb Utah]

This short survey aims to understand social support and social
opportunities of visually impaired and blind persons aged 18-65 years.
There is no compensation for participation however by participating in this
survey you are contributing to research in an area that hasn't been
thoroughly addressed. Note that all responses will remain confidential, but
please do not give any identifying information such as: names, important
dates, etc. You are free to discontinue taking the survey at any time. If
