If you’re talking specifically about ReCAPTCHA, sometimes it happens that (for whatever Googly reason Google has) sometimes Google won’t let you access the CAPTCHA. I can’t remember if it’s audio specifically or any, but I’ve definitely gotten an error where Google does not allow me to access ReCAPTCHA, because…well…it had something to do with suspicion of automated submission activity from my IP address, which was clearly not the case. When this happened to me some few months ago, I couldn’t access this particular ReCAPTCHA-enabled site for several days, until suddenly I could again. Not sure if that’s what Brian’s talking about or not, but that’s been my only problem with ReCAPTCHA thus far. In general, whatever bit of cleverness they’ve done to determine that one is not a robot by the clicking of a checkbox, generally works pretty well. Of course, an audible CAPTCHA isn’t a great solution for people who are deaf-blind, so if the checkbox doesn’t work, it still leaves folks out, but it’s probably the least bad solution to an admittedly real problem.
