This may fall under the umbrella of special circumstances.   For awhile 
David was given to understand that he couldn't take his cane with him on
Shabbat, but then it was recognized that he wasn't carrying it, so much
as it was 
guiding him, and he wouldn't get there otherwise.   The same may apply
to 
wheelchairs, though the Eruv may take care of that.

Yes, there is an umbrella of special circumstances, but a Rav should be
consulted.  When I asked one Rav if I could carry my white cane outside
on Shabbos before we had an eruv he said only if I used it in the house.
I used the cane a few hours a week in the house and went on my merry way
on Shabbos.  He was quoting the Halacha.  Judith

I use a cane occasionally (support cane) if I have to walk long
distances.  
 This means a mile or more.   However, I don't use it in the house.
It's 
easier to sit down if I get tired.   Not so easy on the walk to shul.   
Consequently, I don't get there very often.
