Hello All,

You are invited to participate in a doctoral research project at the University of Wisconsin Milwaukee on evaluating factors that are most important to individuals with impairments when they purchase and use medical devices. To participate in this study you have to be at least 18 years of age. As a participant in this study, you can enter into an I-pod raffle, the results of which will be released in April 2010. The information obtained from this survey will enable us to provide information to medical device designers and manufacturers about features that should be included in medical devices so they are usable by persons with disabilities.

To access this survey, click on the following link

As you are completing this survey, if you need to take a break, click on the link on the top right of the screen "Save and continue later". Once you do this, you will be provided with an alternate link in your email with your saved information and you can access that at any point to complete the survey. Your participation in this study is completely voluntary. There are no foreseeable risks associated with this survey. However, if you feel uncomfortable answering any questions, you can stop the survey at any point.

It is very important for us to learn your opinions to better improve medical device design for people with disabilities. Your survey responses are completely anonymous. Your information will not be shared with anyone outside the research project. All information from the survey will be presented as an aggregate, no individual responses will be used. If you have any questions related to the survey, please call Rochelle Mendonca at 414-229-6803 or email her at r2d2-mendonca at uwm.edu

Thank you for your interest and participation.
