Basically you need:
Certified Birth Certificate. It must be certified- if you were born in Missouri you can get this from any local health department.
Social Security Card
Two proofs of residence such as mortgage statement, bank statement, utility bill.
Certified Marriage license if name changed due to marriage. -obtained in Missouri from the recorder of deeds in the county of marriage. 
The word certified  is critical as a lot of birth certificates and marriage licenses are not certified. It will have the word certified and likely a raised seal.
As for requesting a replacement Social Security Card. If you have an active My Social Security account you may be able to request it on-line  depending on what you've done with it. But, from personal experience, you cannot create an account and then request a replacement card. So, in my case, I'll have to go into the office.

Carol and Elisabeth got their Real ID's this month. You do have to ask for them otherwise they will give you the old type. I was not able to get one because back 45 years ago I as a teenager lamented my Social Security Card so now sometime I'll have to go the SS office to apply for a replacement. Bureaucracy is so much fun!

I asked shortly after the law was passed that they would be available upon request. I’m not sure if he just wasn’t aware, though he was quick to inform me of the extension, or if he was just resistant about giving them out. Thank goodness I have a passport but an I’d is much easier to carry with you.
